If you would like to receive your monthly proceeds in the form of an electronic funds transfer (ACH) please print and fill out the Authorization Form 
, sign and date it and mail it to us along with a voided check from the account which will be receiving the transfers. Any funds due to you will be sent to this account on the 6th of each month or the next business day if that date is a weekend or holiday.
